Whether you are expecting a newborn, have recently moved to our area, or are looking for a new medical home for your children, we thank you for trusting us with your child’s health! We would be delighted to serve as your pediatric health care providers, and we look forward to a long and collaborative relationship with you as your child grows. We welcome new patients starting at birth to age 18 years. Established patients can be seen up to age 21 years.
Are we a good fit?
Choosing a pediatrician for your child is an important decision. It is crucial that your pediatrician’s philosophy on health and wellness aligns closely with that of your family. If you have not done so already, please read our mission statement and the details below to make sure that we are the best fit for you and your family.
Vaccinations
We are a vaccinating practice. Sound Beach Pediatrics requires that all of our patients are immunized according to the current vaccine schedule recommended by the American Academy of Pediatrics.
Insurance
Please verify with your insurer that we participate in your specific plan. Though we may participate with your insurer, some plans are excluded.
Medical Records
Reviewing your child’s medical records is important to us, so that we have the most comprehensive understanding of your child’s health history. Once you have requested records from your prior pediatrician and registered your child with our practice, we will be happy to set up an appointment. Please note the following:
- Well visit appointments may be booked only after we receive your child’s complete medical record.
- Newborns can be registered and seen immediately (after discharge from hospital).
- Sick or urgent visit appointments can be booked as soon as your child is registered with us.
We prefer to receive health records digitally for ease of upload to our electronic health record system.
- Digital files may be sent by email to [email protected] as an encrypted PDF file (Adobe software). Please call our office to provide the password at (203) 363-0123. Encryption is required due to HIPPA regulations.
- Digital files may be uploaded to our HIPPA-compliant AthenaHealth Patient Portal after your child is registered and has had an office visit with one of our providers.
- Faxed records are acceptable once your child is registered with us. Our digital fax # is (475) 619-9855.
We will also accept a hard copy of your child’s health records (delivered in person or sent via mail).
Before your first visit
Before your first visit, we ask that you complete our new patient forms. These may be completed electronically through our digital check-in partner, Phreesia. (You will receive a link from Phreesia prior to your first appointment). We ask that you complete these digital forms prior to your visit to make check-in more efficient for you and to reduce congestion in our waiting room. Our forms are also available for your reference on our Patient Portal.
